If you are thinking about contacting Manchester Paranormal Investigations with a request to investigate, here is a sample form that you can use to log events you feel are paranormal in nature.  This is not mandatory, but can help you recollect and help us see patterns of activity to plan for a successful investigation. 

The most important details are date, time, who witnessed it, and what exactly happened.  The log can be as detailed or brief as you wish – they can be scribbled in a notepad, sheet of paper, or a highly detailed log. 

We also ask our contacts to keep a log of all events during the phase between the different investigation phases. 

What types of occurrences should be logged? 

• Voices
• Smells/Odors
• Moving Shadows
• Orbs
• Apparitions
• Smoky/Misty Forms
• Strong Feelings of Dread or Sorrow
• Cold/Hot Spots
• Rappings, Knockings, or Footsteps
• Door(s), Window(s) Opening 
• Moving/Disappearing Objects 
• Electrical Disturbances 
• Any other event you feel is unusual 

Whenever something like this happens, it is important to note:

• Date of occurrence
• Time of occurrence (include AM/PM) 
• Who witnessed/experienced this
• What room did it occur – where in that room
• What exactly happened
• Did the overall feeling/mood in the room change before/during/after event
